General Description
The purpose of this job is to provide supervision of Preadmission Screening and Resident Review (PASRR) MI & IDD, Enhanced Community Coordination and Diversion Coordination. Responsibilities include monitoring staff and program performance, enforcement of policies and procedures that enable consumers to provide appropriate services as needed and comply with HHSC, accreditation bodies, and payor source requirements. Additionally, allocates time to MFP TST functions as directed by Director of Care Transitions and in alignment with TST Contractor form.
This class works independently, under limited supervision, reporting major activities through periodic meetings.
Duties and Responsibilities
The functions listed below are those that represent the majority of the time spent working in this position. Management may assign additional functions related to the type of work of the position as necessary.
- Supervises, directs, and evaluates assigned staff, processing employee concerns and problems, directing work, counseling, disciplining, and completing employee performance appraisals.
- When deemed needed, provides habilitation coordination to clients who reside in nursing facilities and receive services through the PASSR, Medicaid Waiver and/or non-waiver programs, which include monitoring and oversight of client care.
- Coordinates, assigns and reviews of work and establishes work schedules; maintains standards; monitors status of work in progress; inspects completed work assignments; answers questions; gives advice and direction as needed.
- Provides administrative and clinical supervision to Program Staff to manage day-to-day progress with services provided
- Actively participates in mid management level center-wide and divisional goal/objective setting meetings. Relay operational information between the team and upper management. Coordinates two-way communication of plans and monitors reports outlining achievement. Participates in the design of unit services and training.
- Provides leadership and development opportunities for staff and promotes, enables and executes continuous improvement activities within the department.
- Establishes good working relationships with community groups, funders, and other organizations and represents EHN and division at community activities to enhance, communicate and achieve goals of the organization.
- Effectively completes necessary monthly encounters with all clients on assigned caseload and utilizes the tracking tools necessary to effectively maintain caseload, provide quality client care, meet performance targets, and ensure high-quality, accurate, and timely submission of all documentation.
- Demonstrates the ability to understand and effectively apply all rules and regulations pertinent to the PASSR, ECC, Diversion Coordination, and Medicaid waiver or non-waiver programs.
- Shows dedication to ethical judgment and maintains compliance with all training requirements to effectively accomplish position duties and meet performance outcomes and targets.
- Completes all program required forms in accordance with HHSC and internal timeframes.
- Coordinates educational activities for service coordinators and other EHN staff about available community services and about strategies to avoid nursing facility placement.
- Demonstrates the ability to organize and structure days and times with effective time management skills to successfully fulfill all essential duties.
- Monitors Enhanced Community Coordinator (ECC) caseload to not exceed no more than 30 clients.
- Monitors ECC duties to include pre- and post-transition services, and monthly monitoring to not exceed 365 days after diversion or transition unless additional support is identified; transition planning, linking, and collaboration amongst service coordinators, enrollment specialists, SSLC staff, nursing facility staff, habilitation coordinators, and providers.
- Efficiently works cohesively with program Director and in conjunction with other LIDDA Supervisors, Managers, & Directors to enhance delivery of programs and submit requested data and reports by the target date.
- Meets regularly with assigned staff, maintains outlines of meeting topics and establishes a positive, healthy and safe work environment.
- Maintains and ensures confidentiality and privacy of division and consumers per Protected Health Information Standards and Federal Laws.
- Implement a performance management process for all associates to ensure that all programs performance standards, productivity targets, Program Plan completions, Lag time compliance, and other targets as assigned, are monitored on an ongoing basis. Formulates improvement plans to evaluate professional development and Inspire Change Standards Alignment (Safety, Innovation, Communication, and Efficiency) in the workplace in collaboration with the Care Transition Director.
- Obtains and maintains all credentialing requirements as a QIDP as required by LIDDA Performance Contract with HHSC.
- Attend Transition Support Team multidisciplinary case consultations and provide their expertise in their scope of practice to review all client documentation and provide suggestions in multidisciplinary and peer review case consultations, and review educational information provided to the public as directed by Care Transition Director.
- Demonstrate ability to use good clinical judgment, get along well with others, and work effectively in a fast-paced setting.
- Demonstrated computer and documentation skills.

Minimum Education and Experience Requirements
Requires a Bachelor's degree, Masters degree preferred in Psychology, Social Sciences or related behavioral field; Supplemented by at least two (2) years of MH/ IDD local or other non-profit medical provider entity and one (1) year of supervisory experience; or possession of any equivalent combination of education, training, and experience which provides the requisite knowledge, skills, and abilities.
Experience with trauma-informed services; cognitive behavioral therapies, including Dialectical Behavioral Therapy (DBT); and motivational therapies including the use of incentives, preferred.
Required Knowledge and Abilities
Knowledge of trauma-informed theories, principals, and practices (includes multi-faceted understanding of concepts such as community trauma, intergenerational and historical trauma, parallel processes, and universal precautions), preferred.
Physical Demands
Performs sedentary work that involves walking or standing some of the time and involves exerting up to 10 pounds of force on a regular and recurring basis or sustained keyboard operations.

Special Certifications and Licenses
- Must be able to be credentialed as a Qualified Intellectual Development Disability Professional (QIDP) within the timeframe designated by EHN.
- Provisional license preferred.
- Must possess and maintain a valid state Driver's License with an acceptable driving record.
- Must be able to pass a TB, criminal background and drug screen.
- Bilingual (English/Spanish) Preferred.
