General Description

The purpose of this job is to provide assessments for residents of nursing facilities and those requesting admission when there is reason to suspect the individual may have a developmental disability. The purpose of these assessments is to determine if specialized services related to developmental disability should be provided to the resident or if the individual requesting admission or newly admitted to a nursing facility could be served in a less restrictive setting in the community, diverting them from nursing facility admission.

These duties will also involve providing habilitation coordination to a caseload of residents who are residing in nursing facilities to assure that appropriate services are delivered while they remain in the setting and assisting them to consider community living options if their needs can be met in a less restrictive setting. The ability to work collaboratively with others, including nursing facility personnel, agency provider staff and families will be required. Services will include face to face contact with individuals on the caseload monthly. Expectations and requirements will follow the LIDDA Performance contract with HHS.

This class works independently, under limited supervision, reporting major activities through periodic meetings.

Duties and Responsibilities

The functions listed below are those that represent the majority of the time spent working in this position. Management may assign additional functions related to the type of work of the position as necessary.

  • Provides habilitation coordination to clients who reside in nursing facilities and receive services through the PASSR, Medicaid Waiver and/or non-waiver programs, which includes monitoring and oversight of client care.
  • Responding to referring entity within 72 hours for Pre-Admission Screening process or to the Nursing Facility within 7-14 business days for the Expedited Admission or resident Review processes to complete Level II evaluations
  • Responsible for daily monitoring of and entering completed assessments into the TMHP LTC portal
  • Actively participating and facilitating treatment team meetings, including IDT's & SPT meetings in accordance with state requirements
  • Provides or arranges for the provision of educational or information activities addressing community living options for individuals in nursing facilities and their families;
  • Coordinates educational activities for service coordinators and other EHN staff about available community services and about strategies to avoid nursing facility placement;
  • Complies with habilitation/service coordination staff training requirements as per Texas Administrative Code and LIDDA Performance Contract with HHSC
  • Obtains and maintains all credentialing requirements as a QIDP as required by LIDDA Performance Contract with HHSC
  • Demonstrate ability to use good clinical judgment, get along well with others and work effectively in a fast-paced setting
  • Demonstrated computer and documentation skills

  • Effectively completes necessary monthly encounters with all clients on assigned caseload and utilizes the tracking tools necessary to effectively maintain caseload, provide quality client care, meet performance targets, and ensure high-quality, accurate, and timely submission of all documentation.
  • Demonstrates the ability to understand and effectively apply all rules and regulations pertinent to the PASSR, Medicaid waiver or non-waiver programs, including proper completion of all documentation.
  • Shows dedication to ethical judgment and maintains compliance with all training requirements to effectively accomplish position duties and meet performance outcomes and targets.
  • Completes all program required forms in accordance with HHSC and internal timeframes and ensures all progress notes are entered within 48 hours at a rate of 100%.
  • Effectively organizes and facilitates timely meetings with clients, LARs, interdisciplinary teams to engage in their client's service planning, including reviewing, assessing, and monitoring services. May attend occasional guardianship court hearings for clients.
  • Attends all team and individual meetings as required by supervisor and maintains open communication regarding assigned caseload or such matters that affect the delivery of services to all clients.
  • Demonstrates the ability to organize and structure days and times with effective time management skills to successfully fulfill all essential duties.
  • Maintains flexibility and keen judgment to facilitate effective crisis prevention and management, linking and assisting the clients to secure appropriate and accessible services and supports.
  • Performs other duties as assigned.

Minimum Education and Experience Requirements

Required Bachelor's Degree in Psychology, Social Work, Counseling or related subject. Preferred Master's Psychology, Social Work, Counseling or related subject. Supplemented by at least two (2) years of MH/ IDD local or other non-profit medical provider entity and one (1) year of supervisory experience; or possession of an equivalent combination of education training and experience which provides the requisite skills and abilities.

Experience with trauma-informed services; cognitive behavioral therapies, including DBT; and motivational therapies including the use of incentives, preferred.

Required Knowledge and Abilities

Knowledge of trauma-informed theories, principals, and practices (includes multi-faceted understanding of concepts such as community trauma, intergenerational and historical trauma, parallel processes, and universal precautions), preferred.

Physical Demands

Performs sedentary work that involves walking or standing some of the time and involves exerting up to 10 pounds of force on a regular and recurring basis or sustained keyboard operations.

Involves routine and frequent exposure to:

  • Bright/dim light; Dusts and pollen.
  • Other extreme hazards not listed above.

Special Certifications and Licenses

  • Must be able to be credentialed as a QMHP-CS in the timeframe designated by EHN.
  • Must possess and maintain a valid state Driver's License with an acceptable driving record.
  • Must be able to pass a TB, criminal background and drug screen.
  • Provisional license preferred.
  • Bilingual Preferred (English/Spanish)
