General Description
Oversees the infrastructure that coordinates IDD Intakes, Interest List Maintenance, Enrollments into the HCS & TxHmL Waiver, ICF Placement, Non-Waiver (MCO referrals) eligibility determination, General Revenue eligibility determination and enrollments for individuals with IDD or a related condition. In addition, oversees the infrastructure of State Supported Living Center admissions and transitions, Nursing Facility Diversions and transitions, and Crisis diversions into the HCS waiver. Also, oversees coordination of Community Living options for individuals residing in a State Supported Living Center, Permanency Planning for individuals in a residential facility that are between the age of 16-22, and Transition planning with the county school districts. This infrastructure includes adequate medical, behavioral, acute/chronic, and social care services between internal and external networks.
Duties and Responsibilities

Minimum Education and Experience Requirements
Requires a Bachelor's Degree in a social, behavioral, or human service field including, but not limited to, psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human development, gerontology, educational psychology, education, and criminal justice supplemented by two (2) years of professional experience in an MH/IDD setting and one (1) year of supervisory experience; or possession of any equivalent combination of education, training, and experience which provides the requisite knowledge, skills, and abilities.
Experience with trauma-informed services; cognitive behavioral therapies, including DBT; and motivational therapies including the use of incentives, preferred.
Required Knowledge and Abilities
Knowledge of trauma-informed theories, principals, and practices (includes multi-faceted understanding of concepts such as community trauma, intergenerational and historical trauma, parallel processes, and universal precautions), preferred.
Physical Demands
Performs sedentary work that involves walking or standing some of the time and involves exerting up to 10 pounds of force on a regular and recurring basis or sustained keyboard operations.
