General Description

The purpose of the Community Health Worker (CHW) Program Supervisor is to ensure program and contract rules are followed in the assigned program while adhering to company rules and budget guidelines and for supervision of CHW's.

This class works under general supervision, independently developing work methods and sequences.

Duties and Responsibilities

The functions listed below are those that represent the majority of the time spent working in this position. Management may assign additional functions related to the type of work of the position as necessary.

  • Supervises, directs and evaluates assigned staff, processing employee concerns and problems, directing work, counseling, disciplining and completing employee performance appraisals.
  • Spends, at a minimum, fifty percent (50%) of work time delivering direct participant services which meet this contract performance measures.
  • Participate on programmatic conference calls as scheduled by System Agency.
  • Coordinates, assigns and reviews work and establishes work schedules; maintains standards; monitors status of work in progress; inspects completed work assignments; answer questions; gives advice and direction as needed.
  • Establish and maintain working linkages through Memorandums of Understanding (MOU's) with a network of community and social service agencies serving or having an interest in the eligible population.
  • Engage in assertive outreach in locations and times where the eligible populations are likely to be found.
  • Performs other duties as assigned.

Minimum Education and Experience Requirements

Requires Bachelor's Degree in Health or Human Services, Social Services, Rehabilitation, Emergency Management or related field, supplemented by one (1) year of supervisory experience; or possession of any equivalent combination of education, training, and experience which provides the requisite knowledge, skills, and abilities.

Required Knowledge and Abilities

  • Knowledge of trauma-informed theories, principals, and practices (includes multi-faceted understanding of concepts such as community trauma, intergenerational and historical trauma, parallel processes, and universal precautions), preferred.
  • Be knowledgeable and competent in discussing HIV, HCV and other communicable diseases associated with substance use and be able to demonstrate ability to discuss sexuality openly and comfortably.
  • Be knowledgeable and competent in discussing opioid overdose and be able to demonstrate ability to train individuals to use overdose reversal medication and harm reduction materials.

Physical Demands

Performs sedentary work that involves walking or standing some of the time and involves exerting up to 10 pounds of force on a regular and recurring basis or sustained keyboard operations.

    • Electrical shock; Heights.
    • Radiation; Disease/pathogens.
    • Toxic/caustic chemicals; Explosives; Violence.
    • Other extreme hazards not listed above.

Special Certifications and Licenses

  • Requires CHW's to obtain DSHS Community Health Worker certification within 6 months from date of contract or start of date of employment.
  • Licensed Chemical Dependency Counselor preferred, but not required.
  • Must possess and maintain a valid state Driver's License with an acceptable driving record.
  • Must be able to pass a TB, criminal background and drug screen.
