General Description
The purpose of this position is to be a part of a multidisciplinary team composed of a Psychologist, a Medical Doctor, a Psychiatrist, a Registered Nurse, and an LPC and provide support services through case consultation, training and education on specific topics related to IDD and mental health, behavioral, and/or medical comorbidities. Support services for individuals with an Intellectual Developmental Disabilities (IDD) diagnosis are geared toward preventing institutionalization to state supported living centers or nursing facilities. The Lead TST Coordinator collaborates and supports Money Follows the Person (MFP) Federal Demonstration HUB Region 1 that includes El Paso, Permian Basin, West Texas Center, and Concho Valley. The Lead TST Coordinator will provide support activities to community staff who services individuals with IDD at risk of admission to an institutional setting and to those who have moved from an institutional setting, including SSLCs, NFs, and medium or larger ICF/IIDs. Community service staff include: Local Intellectual and Development Authorities (LIDDAs), Local Mental Health Authorities (LMHAs), Local Behavioral Health Authorities (LBHAs) HCS & TxHmL Program Providers, Community Living Assistance Support Services (CLASS) Program Providers, Deaf Blind with Multiple Disabilities (DBMD) Program Providers, Designated Service Areas (DSAs), local community providers including, but not limited to, first responders.; and other programs determined by the contracted LIDDA and approved by HHSC as providing support activities to individuals with IDD and appropriate for assistance by the TST.
This position will also qualify under the provision of the Public Service Loan Forgiveness (PSLF) program. The individual will also have access to support with licensure renewal and continued education units (CEUs).
The incumbent will report directly to the Director of Care Transition and will work under general supervision, independently developing work methods and sequences.
Duties and Responsibilities
The functions listed below are those that represent the majority of the time spent working in this position. Management may assign additional functions related to the type of work of the position as necessary.
- Create educational opportunities, such as webinars, conferences, training, and specified correspondence, focused on increasing the expertise of LIDDA and provider staff in supporting the individuals with IDD at risk for institutionalization or those who have moved from institutions (SSLCs, NFs, and medium or large ICF/IIDs).
- Provide technical assistance, upon request from LIDDAs and providers, on specific disorders and diseases, with examples of best practices and evidence-based services for individuals with significant medical, behavioral and psychiatric challenges.
- Facilitate case-specific peer review or multidisciplinary consultations to support service planning teams that need assistance planning and providing effective care for an individual and provide the suggestions within a week to the individual who made the referral for the case consultation.
- Ensure the provision of support activities to all local authorities, waiver providers, and community members, as appropriate, within their DSA, who serve individuals with IDD at risk of admission into an institutional setting and to those who have moved from an institutional setting, including SSLCs, nursing facilities, state hospitals, and medium or large Intermediate Care Facilities (ICF/IIDs). Support activities include technical assistance, educational opportunities, and case reviews;
- Complete required HHSC trainings.
- Schedule and facilitate a stakeholder committee meeting at least annually.
- Schedule, facilitate, and attend monthly TST office hour with the other TST leads to discuss challenges, successes, upcoming educational opportunities, share community resources, and collaborate on program initiatives;
- Develop or facilitate and request pre-approval of new educational opportunities per TST performance measures;
- At least once per quarter, communicate face to face, either in person or through a virtual platform, with the DSA LIDDAs and providers to inform them of the availability of all program services and outlines how to request a service;
- Develop a TST program plan, to be submitted annually to HHSC for review and approval in SharePoint;
- Facilitate the development of the annual contractor form in collaboration with LIDDA leadership and financial staff and monitor budget expenditures.
- Be responsible for the accuracy and timely submission of all quarterly reports, knowledge gained in IDD as evidenced by feedback collected via post-training survey.
- Coordinates with Local Intellectual Disability and Developmental Authority (LIDDA) within the Hub to facilitate multidisciplinary case consults and peer reviews regularly for individuals who are at risk for institutionalization.
- Coordinates with Region 1 Hub to increase educational opportunities, technical assistance, multidisciplinary and peer review case consultations. Educational opportunities will be provided to the community, schools, hospitals, police officers, etc. Technical assistance is a one-to-one service regarding questions the individual may have regarding diagnoses, how to obtain a Determination of Intellectual Disability, behavioral interventions, resources in the community, etc.
- Maintains strong communication with the LIDDAs and providers in the Designed Service Area.
- Out of town Travel to the Designated Service Areas (Concho Valley, PermiaCare, and West Texas) to provide support and/or education/training.
- Out of town travel to attend Conferences for professional development.
- Conduct community outreach to inform agencies of the availability and type of educational opportunities, and outline how to request educational opportunities from their region's LIDDA.
- Collaborates with the Director of Care Transition on the utilization and development of resources to provide training to the LIDDA's catchment area. Training opportunities will be on specific topics requested by the agency, school, caregiver/LAR, etc.
- Explain and provide support in person on the multidisciplinary case consultation suggestions or Behavior Support plans as needed on a case-by-case basis.
- Complete Quarterly reports for MFP Hub Region 1on HHSC quarterly report template. Requests data from the contracted LIDDAs to complete the report.
- Logs and maintains accountability of trainings and peer reviews on a day-to-day basis.
- Completes Annual Satisfaction Surveys in accordance with HHSC requirements.
- Engages with local community leaders to advocate for Money Follow the Person Demonstration and develop partnership activities.
- Coordinates and facilitates MFP HUB meetings on a monthly basis with the contracting LIDDAs.
- Maintains attendance at scheduled statewide MFP HUB Meetings.
- Coordinates and facilitates MFP Advisory Council Meetings on an annual basis with all advisory council members in a combination of modes such as face to face and/or video technology.
- Attends Community Stakeholders committees/meetings as assigned.
- Performs other duties as assigned.
Minimum Education and Experience Requirements
Master's degree in psychology, Social Sciences or related behavioral field, supplemented by two (2) years of experience; or possession of any equivalent combination of education, training, and experience which provides the requisite knowledge, skills, and abilities. Requires a Licensed health-related professional with direct work experience in the IDD field.
Experience with trauma-informed services; cognitive behavioral therapies, including Dialectical Behavioral Therapy (DBT); and motivational therapies including the use of incentives, preferred.
Required Knowledge and Abilities
Knowledge of trauma-informed theories, principles, and practices (includes multi-faceted understanding of concepts such as community trauma, intergenerational and historical trauma, parallel processes, and universal precautions), preferred.
Physical Demands
Performs sedentary work that involves walking or standing some of the time and involves exerting up to 10 pounds of force on a regular and recurring basis or sustained keyboard operations.

Special Certifications and Licenses
- Requires at least one of the following: Licensed Professional Counselor (LPC), Licensed Clinical Social Worker (LCSW), Licensed Marriage and Family Therapist (LMFT), Board Certified Behavior Analyst (BCBA), or Texas Registered Nurse (RN).
- Must possess and maintain a valid state Driver's License with an acceptable driving record.
- Must be able to pass a TB, criminal background and drug screen.
